Analytical
Request Form
IATL can process hundreds of samples each day. These include samples for long-term projects as well as samples received each day for expedited turnarounds. There are several ways to notify the laboratory when submitting samples requiring an expedited turnaround time. These include a telephone call to your Customer Service Representative, faxing an
Analytical Request Form (51k) or filling out an on-line request form in our
Submit Samples section.
Chain-of-Custody (COC)
Each sample submittal must have a Chain-of-Custody (COC)
(63k) document. This
documents the legal custody and history of relinquishing and receiving your
samples. It is also used as an instrument for you to communicate turnaround
times, billing and reporting instructions, analytical methods, etc. IATL
encourages our clients to employ their own documentation and forms. You may
use and duplicate the COC provided here.

Sample Log
Consider using a Sample Log to compliment your chain-of-custody
documentation. This form supplies valuable information to the laboratory
concerning your sample's description, location sampled, dimensions of area
or volume sampled, etc. IATL encourages our clients to employ their own
logs. You may use and duplicate those provided here. Some logs are analyte
and matrix specific.
Credit Application
Establish an account at IATL. Please forward this
Credit Application (53k) to
your company's Accounting Department. Please forward via fax or as an
email attachment.
Contact List
Stay in communication with IATL management and laboratory personnel during
business hours and at off-hours and weekends. The
Contact List is
frequently duplicated and provided to your field sampling technicians. In
this way, emergency numbers are provided to facilitate analyses 24/7/365.

Analytical Methods
Many clients request information on field sampling requirements (e.g. filter
media, flow rates, minimum volumes, etc.) and Analytical Methods.

MDLs / LOQs
Limits of Quantitation (LOQ) vary from Method Detection Limits (MDLs),
Limits of Detection (LODs), and Reporting Limits (RLs) according to
definitions through the American Industrial Hygiene Association and recent
ISO17025 guidelines. IATL performs annual MDL studies for most of its
analytes.

QA/QC
Clients often inquire about our Quality Assurance and Quality Control
Program (QA/QC Program) at IATL. Sometimes sections of our QA/QC SOPs or
other information documenting our performance in proficiencies etc. is
requested by our clients.
