Sample LogConsider using a Sample Log to compliment your chain-of-custody documentation. This form supplies valuable information to the laboratory concerning your sample's description, location sampled, dimensions of area or volume sampled, etc. IATL encourages our clients to employ their own logs. You may use and duplicate those provided here. Some of these forms can be printed, copied, completed, and faxed, or sent with your samples. Some of these forms may also be 'writeable', that is, you can fill them out on-line and email them directly to the laboratory!
